Primary Care Access Point (Guichet d’accès à la première ligne – GAP)
People registered with the GAMF, and therefore awaiting a family doctor only, who have health needs can contact the GAP.
Simply dial 811, option 3 to access this service. The nurse will be able to provide specific advice by phone, then direct the call, if necessary, to the appointment centre for a medical consultation. Users could be referred to health professionals, such as family doctors, nurse clinicians, specialized nurse practitioners, community pharmacists or social workers in the region. Since January, users could also be referred to the new specialized nurse practitioner clinic in the region. Due to the high volume of calls, we are aware that you may be on hold for longer than usual.
The digital GAP
In addition to this telephone line, it is also possible to use the digital GAP. This assessment is used to determine the health service that corresponds to your situation. Please consult the information available online at the following address: gap.soinsvirtuels.gouv.qc.ca/en. You will discover how to obtain an appointment with a health professional or find a service adapted to your needs.
You think you need a consultation?
If your child has a family doctor, call their clinic.
If your child does not have a family doctor, call the Primary Care Access Point at 811, option 3.
If you need advice or are not sure if you should see a doctor, call Info-Santé 811, option 1 (then option 2), to access the priority line for parents of children 0-17 years old. All parents of children between these ages can use this telephone line. During the call, a nurse will first assess the situation and the child’s condition. The nurse may then provide specific advice and refer the call, as needed, to the appointment centre for a medical consultation.
Don’t hesitate to consult your pharmacist for advice on which medication to use and for more health tips.

For people with a family doctor
If you have a family doctor, you must communicate with your doctor to make an appointment. If your doctor is not available and you are registered with a family medicine group (FMG), you can make an appointment with another doctor or health professional in the FMG.

There are options with or without an appointment in certain medical clinics in the region. You can consult the various resources available online to determine the clinics close by that offer walk-in hours, such as the Répertoire en ressources en santé et services sociaux or the Québec Medical Appointment Scheduler.
For minor health problems, you can dial 811 to contact Info-Santé/Info-Social and talk to a nurse or a psychosocial worker.

This single-entry point allows you to talk directly to a qualified worker and gain quicker access to the psychosocial services of the Centre intégré de santé et de services sociaux de Lanaudière.
The Info-Social team assesses and transfers the request to the service or program that is most suitable to the person’s situation.
Psychosocial services are dispensed by professionals for you, your family and your children. These services are customized and adapted to your situation and needs.

Are you on the waiting list for a family doctor?
This service is only offered to people in the Lanaudière region who are on the waiting list for a family doctor or nurse practitioner specialized in primary care.
This ad hoc service will facilitate access to a health professional, if needed. It will provide you with timely access to a health professional to respond to a specific need, but will not assign you a family doctor. The teams will not provide you with long-term medical follow-up.
Note that you will still be on the Québec Family Doctor Finder (GAMF) waiting list until you are permanently assigned a doctor or nurse practitioner specialized in primary care.

Services offered
A nurse will evaluate your request and refer you to the appropriate resource (follow-up with a nurse, doctor, psychologist, etc.) based on an assessment of your clinical condition.
You can also contact Info-Santé at 811 and talk to a nurse or psychosocial worker, depending on your condition.
However, in case of an emergency or if your health condition requires immediate attention, call 911 or go directly to the nearest hospital Emergency department.
- Is your autonomy limited or are you unable to get around easily because of your health condition?

Home support services are offered to clients of all ages (mostly seniors) presenting temporary or permanent disabilities, who wish to remain at home but do not have the mobility to obtain ambulatory services.

The test centre does blood tests and all other sample collections.
For your appointment at the test centre, bring:
- The requisition from your doctor indicating the tests requested
- Your health insurance card
Before your appointment
Make sure you are aware of the instructions related to your test (Do you need to fast? If so, for how long, etc.).
For cholesterol levels (triglycerides), it is recommended that the person fast for at least 12 hours. Check your requisition to find out if this is necessary in your case.
Home testing
The CISSS also offers home testing for people with temporary or permanent disabilities. To find out if you are eligible for this service, contact the home support service at your CLSC.

People who are most at risk of complications and death associated with the flu are encouraged to protect themselves by getting a vaccination.
The following clienteles are being prioritized for the vaccine:
- Anyone 6 months of age or over with a chronic disease (including pregnant women, regardless of stage of pregnancy)
- Anyone 75 years of age or over
- Pregnant women in the 2nd or 3rd trimester
- Informal caregivers or loved ones residing with someone who is at risk or with a baby under the age of 6 months
- Health workers, particularly those dispensing direct care to users in hospital, in a CHSLD or at home

Mobile teams from the CISSS will be visiting several municipalities in the region to offer the flu vaccination.

Pregnancy follow-up
With a physician
You can be followed during your pregnancy by a delivering physician or an obstetrician, depending on your situation. Your delivery will take place at the hospital.

With a midwife
If you are having a normal pregnancy, follow-up can be provided by a midwife with a university degree in midwifery. This is a free service, covered by Québec health insurance.
Your delivery could take place in your home, at Hôpital Pierre-Le Gardeur or at the birthing centre Maison de naissance de Lanaudière.

It is very important to complete all of the sections on the form so that we can process your form quickly and correctly. If you complete the form by hand, please write legibly in block letters. If you have already been followed by a midwife in Lanaudière or elsewhere, you can indicate this in Section 3.
We will contact you in 2 to 6 weeks by phone to confirm if you have a place, or by email if you have been put on a waiting list.
We recommend, however, that you take steps to make an appointment with your physician or gynaecologist in the event that no places are available.

Prenatal information sessions
Services offered Prenatal information sessions, or prenatal classes, are a great way to prepare for the arrival of your baby.
Offered by partner organizations working with families, these sessions help to prepare parents for success during the pregnancy, delivery and baby’s first days. They provide information that will help you make informed decisions about the pregnancy and delivery. And they will equip you with tools to support you in feeding your newborn, adapting to your new role as parent and caring for your newborn.

We recommend that you register as soon as possible since these group sessions begin at the 12th week of pregnancy. You can find out how to register with organizations in your RCM by consulting the section Rencontres prénatales (in French) on this website.
